- A feature printed with thermochromic ink.
- An opaque writing area designed so writing disappears if the prescription is lightened.
- A description of the security features printed on the form.
- Quantity and refill checkboxes, required prescriber details, and the approved security printer’s identifying number.
- A uniquely serialized number, plus batch and sequential numbering requirements.
Serial number and barcode
The California DOJ’s AB 149 guidance explains that compliant controlled substance paper prescription forms use a 12-character serial number and corresponding barcode. These elements help pharmacists and regulators identify and validate individual forms.
Why the approved printer matters
Controlled substance prescription forms must be obtained from a printer approved by the California DOJ. Approved printers are responsible for verifying prescriber licensure and prescribing privileges before printing forms and must follow controlled delivery and recordkeeping procedures.
